Initial assesments

Long or extended consultations for thirty to forty-five minutes are necessary for new patients with complex medical histories.

We ask you to provide some information about yourself, your medical history and the conditions you want treated.  This will help us better understand your needs and expectations.

If you want to complete the forms at the surgery, please arrive ten minutes before your scheduled appointment time.

Please remember to bring copies of your previous test results and specialist letters if available.
